Intraoperative dorsal cochlear nucleus action potential monitoring in hearing preservation retrosigmoid surgery for vestibular schwannoma: a predictive analysis of short-term hearing outcomes

A dispatch from PubMed — filed

The aim of this study was to evaluate the safety and short-term predictive value of continuous intraoperative monitoring of the dorsal cochlear nucleus action potential (DNAP) using a 4-contact depth electrode during retrosigmoid vestibular schwannoma resection, and to compare its accuracy in predicting postoperative hearing outcomes with that of standard auditory brainstem response (ABR).

✦ Clinical Takeaway ✦

Dorsal cochlear nucleus action potential monitoring during retrosigmoid vestibular schwannoma surgery may offer predictive value for short-term hearing preservation, but evidence is preliminary; no change to standard intraoperative monitoring protocols is warranted until larger validation studies are available.

✦ Why It Matters ✦

If confirmed in larger cohorts, this monitoring technique could give surgeons real-time feedback to improve hearing preservation rates in vestibular schwannoma surgery.

✦ Key Points ✦
  1. 01Evaluated dorsal cochlear nucleus (DCN) action potential monitoring as a hearing-outcome predictor during hearing-preservation surgery.
  2. 02Surgery type was retrosigmoid approach for vestibular schwannoma removal.
  3. 03Focus is on short-term (not long-term) hearing outcomes post-operatively.
  4. 04Adds to the small but growing literature on advanced intraoperative neurophysiological monitoring for skull-base surgery.
  5. 05Published in the Journal of Neurosurgery (JNS), a peer-reviewed surgical journal.
✦ Claims & Evidence ✦

Intraoperative DCN action potential monitoring can predict short-term hearing outcomes in hearing-preservation retrosigmoid surgery for vestibular schwannoma.
